When We Are Closed
If you require urgent medical assistance which cannot wait until the surgery is open please dial
111.. Calls to the NHS 111 service are free from both landlines and mobiles.
If you have a life threatening medical emergency please dial 999. Chest pains and / or difficulty in breathing constitute an emergency.
Alternatively patients can visit one of the Walk In Centres based in Harold Wood, Dagenham, Barking and Rainham.
Harold Wood Polyclinic is open from Monday to Sunday 8am-8pm all year (including bank holidays). Location: The Drive (off Gubbins Lane), Harold Wood, RM3 0AR. Tel 01708 792000.
The Broad Street Clinic is open Monday to Friday 7 a.m. to 10 p.m. at weekends and bank holidays from 10 a.m. to 6 p.m.
The Upney Lane Clinic is open from 9 a.m. to 7 p.m. daily.
The Mardyke Clinic is open from 10 a.m. to 2 p.m. and from 3 p.m. to 7 p.m. 5 days a week and 10 a.m. to 2 p.m. at weekends (not Bank Holidays).

From Monday 30th March 2015 you shall be able to see a GP locally from 6:30pm-10:00pm on weekdays and 12.00pm-6.00pm on Saturday and Sunday please click on attached link for more info.
To register at this surgery you must meet the following criteria:
Live within the practice area – ask at reception
Provide proof of identification e.g. driving licence, passport, birth certificate, current visa or identity card
Provide proof of address e.g. utility bill, council tax bill, bank statement (dated within the last 3 months)
Mobile phone invoices will not be accepted.
Patients under the age of 7 years will need to provide a birth certificate and immunisation history (red book)
Complete ALL the forms
This includes the GMS1 form and the New Patient Questionnaire
Failure to do this will result in an unsuccessful application.
Once you have completed all of the above, please hand it back to the receptionist. The surgery will then handle your application
If your application is unsuccessful you will be contacted and your paperwork will be left at reception for collection.

If your long-term medication has been approved on a repeat prescription basis you can order in the following ways:
By hand/post - bring or send your computer slip with the required items clearly marked. If you want the prescription posted back please enclose a stamped addressed envelope.
Electronically - to register for requesting your repeat medication electronically please bring in photo ID to Reception and your access will be activated.
Nominated Pharmacy - Order repeat prescriptions via your nominated Pharmacy.
You can also order your repeat prescription directly through this site. Click on the link above and complete the simple registration form.
* Please note we do not take telephone calls for repeat prescriptions as this can lead to errors.
Patients on repeat medication will be asked to see a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications and notification should appear on your repeat slip. Please ensure that you book an appropriate appointment to avoid unnecessary delays to further prescriptions.
