Modern Medical Centre welcome new patients, before filling out an online registration form, check you are in our catchment area. (We do not accept patients outside of our catchment area)

When you register, it's helpful to have your NHS number. You can use the NHS website to find your NHS number.
You will also be required to complete a new patient questionnaire, please download the form below and email or return to the practice. If you are registering a child please use the child questionnaire form.
